These should be filled in when a dog comes in for an in-patient dental procedure. The teeth which have been removed or operated on should be coloured in.
 
Photographs should be taken before and after the procedure and stuck on the right-hand side of the inside of the card.
 
The dental record card should be handed to the client along with the Get Well Soon card when they collect their pet.
